INSURANCE:
All Homeopathic Visits are due in full at the time of service. We accept cash and/ or credit card only. We also request that you provide us with your credit card information to make paying for your service effortless. No insurance is taken or filed.

CANCELLATION POLICY:
Appointments that are cancelled 48 hours in advance will not be charged a cancellation fee of $150.00. This will be charged to your credit card on file. If no credit card is on file you will be billed accordingly. Your signature below indicates you acknowledge this policy and agree to abide by its terms.

HOMEOPATHIC PATIENT CONSENT
CONSENT TO TREAT A CHILD (UNDER THE AGE OF 18 YRS)
I give permission for Dr. Greg Fihn/Maria Fihn, RN, APH, FBIH, Dihom, to treat my child/children with homeopathy for the chronic symptoms that my child is presenting with at this time. I have been informed and understand that Maria Fihn, RN, APH, FBIH, Dihom, does not prescribe medication or perform any diagnostic testing, and does not treat acute problems such as injuries, ear aches, infections, cold/flu’s or emergencies, that my child might have. For any problem that might require conventional medicine, I will be referred to my Primary Care Physician of record. I understand that it is not recommended to discontinue any medications prescribed by another physician for my child. In the event of an emergency with my child, I am to immediately call 911, go to an emergency care center and contact my Primary Care
Physician.
